Child Qualified Professional needed to provide Targeted Case Management to private Mental Health company in Lenoir/Morganton. Must have a bachelors degree with minimum 2 years experience with children in the Mental Health field. Provides the functions of case management: (1) Case Management Assessment, Person Centered Planning, Referral / Linkage; and Monitoring / Follow-Up; (2) Provide after-hour crisis intervention as assigned; Maintain/update consumer medical record forms as necessary; (3) Complete all paperwork necessary including ITRs, PCP's, NC TOPPS; Travel to and from various locations as determined by the client’s needs; (4) Transportation of client to and from appointments as necessary.
